I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH HSL GRIDIRON GAUNTLET IN SUPPORT OF EZRAH, including, but not limited to, any and all injuries that may occur.

More specifically;

In consideration of being allowed to participate in the HSL GRIDIRON GAUNTLET athletics/sports program, and related events and activities, the participant:

Agrees that the participant should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.

Represents that they are physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by a qualified medical professional. Furthermore, participant certifies that there are no health-related reasons or problems which preclude my participation in this activity.

Acknowledges and fully understands that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.

Assumes all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.

Consents to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.

To the fullest extent permitted by law, releases, waives, discharges, and covenants not to sue HSL GRIDIRON GAUNTLET or EZRAH, its affiliated clubs, their respective administrators, directors, agents, and other employees or volunteers of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct the event, all of which are hereinafter referred to as "releases," from any and all liability to each of the participants, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.

To the fullest extent permitted by law, defend, indemnify, and hold harmless THE HSL GRIDIRON GAUNTLET and EZRAH, their agents, officers, and employee of any of them from and against claims, damages, losses and expenses, including but not limited to reasonable attorney's fees and costs, arising out of or resulting from the participants participation in the tournament, regardless of whether or not such claim, damage, loss or expense is caused in part by a party indemnified hereunder.

Agrees that while participating in this activity, participant may be photographed. Participant agree to allow any and all photos, videos, or film likeness to be used for any purpose, in perpetuity, by the activity holders, producers, sponsors, organizers, and assigns.
